For Upcoming Preventive Care Visits

To ensure a seamless experience for your upcoming preventive care visit, be sure to have the following information and documents on hand:

• Good Faith Estimate (For Mammograms and Colonoscopies): This estimate provides an anticipated cost breakdown if pre-payment is required, or costs are variable.

• Provider Information (For all Preventive Visits):

  • Provider Name

  • Provider Phone Number

  • Provider Zip Code

  • Date of Service

After your visit, remember to request the following document to submit with your sharing request:

• Itemized Statement: This outlines the services provided by your healthcare professional along with their associated costs.

 

Requesting Medical Advocacy

As a member of Zion HealthShare, you have access to our complimentary Medical Advocacy service. Our team can help you locate affordable medical providers, negotiate self-pay discounts, arrange pre-payment for upcoming services, and facilitate payment for post-procedure therapy services. For Past Preventive Care Visits

If you've recently completed a preventive care visit and wish to submit a sharing request, simply upload your request and all necessary documents through your Member Portal.

 

Important Reminders

Please submit all sharing requests and related documents within six months of the service date for your request to be considered.

Related Member Guidelines: Preventive Sharing

Preventive Sharing is automatically included in every Direct Membership and can be added to any Essential Membership. This feature supports your health by covering Annual Provider Visits (APV), child wellness exams, youth immunizations, and early detection procedures. We encourage all members to prioritize their health by attending these annual visits and participating in relevant cancer screenings.

While Zion HealthShare typically focuses on sharing costs related to unexpected medical events, we also promote proactive health measures. Our Preventive Sharing feature helps cover certain preventive services to support you and your family’s long-term well-being. Preventive Sharing does not require you to meet your IUA (Initial Unshareable Amount).
